Emerging evidence demonstrates improved treatment adherence may be achieved through shared decision making. Shared decision making is a process in which patients and providers work together to select treatment plans and services. This process evolves over time and supports wellness.

Shared decision making allows patients to make informed choices by balancing information and treatment options with individual preferences and goals.

This program contains a series of interactive role-modeling videos along with unbranded educational resources intended to be used or shared by health care professionals with patients and their caregivers. The program helps individuals understand treatment adherence challenges in mental health conditions and provides practical shared decision making resources to help address those challenges.

Program objectives:

  • Emphasize the importance of treatment adherence and shared decision making in mental health
  • Promote wellness and adherence to treatment plans
  • Enhance communication among all stakeholders
  • Support stakeholders with provider and patient educational materials

This disease management program supports providers, patients, and caregivers.

Health Care Providers

Program materials include key issues for health care providers such as shared decision making, language normalization, goal setting and tracking, and potential strategies to help improve communication with patients. An interactive video series targeted to providers and patients to help address shared decision making and treatment plan adherence through role modeling scenarios is also offered.

Patients* and Caregivers

Health care professionals may choose to share program materials with patients and their caregivers. Program materials for patients and caregivers focus on the importance of shared decision making in treatment adherence, goal setting and tracking, wellness, and communicating with providers. These patient and caregiver resources are also available in Spanish. An interactive video series targeted to providers and patients to help address shared decision making and treatment plan adherence through role modeling scenarios is also offered.
